Below you will find the application form to become a member of Sovicos. Please check your details carefully while filling in the form and do not forget to print it out (double-sided) and place your signature. Unfortunately, we cannot welcome you as a member without your signature.
Before you fill in the application form, make sure you have the following to hand:
- Bank card
- Photo (digital, does not have to be an official passport photo)
- No side views, no faces from too great a distance, no caps or sunglasses and no photo with your BFF. Make sure the photo has sufficient contrast
- If necessary: digital copy of your student card or proof of
- If necessary: your Ooievaarspas
- If available: your Nevobo relationship code
- If available: the number of your (old) Sovicos competition shirt
